COVID-19 Testing Consent Form

Please use this form to provide consent for the COVID-19 testing provision being provided by The Garibaldi School.

Please complete this form once for each of your children that attend the school.

How to do your COVID-19 Test Testing handbook

This form is used to collect information regarding COVID-19 testing and will only be used for this purpose. We need the information collected here to enable us to manage this process effectively. The information is for internal use only and will be stored on our internal servers. This information will be kept in line with our data retention policy.

We will only use your contact details to share information with you that is relevant to this process.

3. Consent

Please read the following statements carefully, then tick to confirm that you consent to all of the statements.

  1. I have had the opportunity to consider the information provided by the school/college about the testing, ask questions and have had these answered satisfactorily, based on the information presented and the information in the documents provided above.
  2. I have discussed the testing with my child and my child is happy to participate. If on the day of testing they do not wish to take part, then they will not be made to do so and consent can be withdrawn at any time ahead of the test.
  3. I consent to my child having a double nasal swab for a lateral flow test.
  4. I consent that my child's sample(s) will be tested for the presence of COVID-19.
  5. I understand that if my child's results are negative on the lateral flow test I will not be contacted by the school/college.
  6. If the lateral flow test indicates the presence of COVID-19, I will be contacted by the school/college.
  7. I consent that they will need to self-isolate following a positive lateral flow test result, until the results of a confirmatory PCR have been received.
  8. I understand it is my responsibility to organise a PCR test.
  9. I understand if the PCR test is taken within 2 days of the positive lateral flow test and is negative, it overrides the lateral flow test and, upon receipt within school/college of the confirmed result in writing, my child can return to school as long as they do not have COVID-19 symptoms.
  10. I agree that if my child's test results are confirmed to be positive from this PCR test, I will report this to the school/college and I understand that my child will be required to continue to self-isolate following public health advice.
